Sliding Fee Discount Program
The Sliding Fee Discount Program may reduce the cost of eligible services based on household income and family size. Approval is not guaranteed, and eligibility depends on documentation, household size, income, and program rules.
What Is the Sliding Fee Discount Program?
Meadowlark Mind & Body offers a Sliding Fee Discount Program to help reduce financial barriers to psychiatric mental health care for eligible patients. Discounts apply only after an application is reviewed and approved by our billing team.
Who May Apply?
Patients who may have difficulty paying for care may apply. Eligibility is based on household size, income, and supporting documentation. Patients with insurance may still ask about sliding fee eligibility when allowed by plan rules.
Required Documents
- Government-issued photo ID
- Household size information
- Proof of household income (pay stubs, tax return, benefit statements, or other documentation as requested)
- Completed sliding fee application
How to Apply
Call or text 307-300-5885 and ask about the Sliding Fee Discount Program.
Our billing team provides the application and explains what information is needed.
Income and household documentation are reviewed with your application.
If approved, you are notified of your discounted rate before eligible services are billed at that rate.
Important Information
Submission of an application does not guarantee approval. Discounts cannot be applied until eligibility has been reviewed and approved.
Some third-party costs, labs, medications, specialty services, supplies, or other charges may not qualify for sliding fee discounts.
Approved discounts may need to be renewed when income, household size, insurance coverage, or financial circumstances change.
The program does not replace insurance verification, medical-necessity requirements, or prior-authorization rules.
