Billing Help and Sliding Fee Discount Program

Meadowlark Mind & Body is committed to improving access to mental health and wellness care. For eligible patients, we offer a Sliding Fee Discount Program that may reduce the cost of certain services based on household income and family size.

Patients may contact our billing team or clinic number to request a sliding fee screening, ask questions about payment options, or learn what documentation may be needed.

About the Sliding Fee Discount Program

Our Sliding Fee Discount Program is available to eligible patients based on household income and family size. Eligibility is reviewed individually after the required application and supporting documentation are submitted.

The program is designed to help reduce financial barriers to care. Approval is not guaranteed, and discounts cannot be applied until the application has been reviewed and approved. Approved discounts may apply only to eligible services and may need to be renewed periodically or when income, household size, or insurance coverage changes.

Services That May Qualify

The Sliding Fee Discount Program may apply to eligible services, including:

  • Psychiatric medication management appointments
  • Therapy initial assessments
  • Established patient therapy visits
  • Other eligible outpatient behavioral health services, when approved

Some services may not qualify. Certain services, medications, labs, specialty treatments, supplies, or third-party costs may not be eligible for sliding fee discounts.

Insurance and Sliding Fee Eligibility

Sliding fee eligibility is based on household income and family size. Patients with insurance may still ask about sliding fee eligibility when allowed by the insurance plan and applicable rules. Some insurance contracts may limit the ability to apply discounts to patient responsibility amounts.

Medicare, Medicaid, commercial insurance, and other coverage rules may vary. Our billing team can review available options and explain what may or may not apply.

How It Works

  1. 1 Contact Meadowlark Mind & Body. Call or text 307-300-5885 and ask about the Sliding Fee Discount Program or billing help.
  2. 2 Complete the Application. Our billing team will provide the application and explain what information is needed.
  3. 3 Submit Income and Household Documentation. You may be asked to provide documentation such as recent pay stubs, a tax return, benefit statements, or other proof of income.
  4. 4 Eligibility Review. Our billing team will review your household size, income, documentation, and eligible services.
  5. 5 Receive a Determination. If approved, you will be notified of your discounted rate or discount level before eligible services are billed at that rate.

Documents You May Need

  • Government-issued photo ID
  • Household size information
  • Proof of household income
  • Three most recent pay stubs
  • Prior year tax return
  • Social Security, disability, unemployment, or benefit statements
  • Other income documentation, if requested

Important Information

Submission of an application does not guarantee approval. Discounts cannot be applied until eligibility has been reviewed and approved by our billing team.

Payment may still be required at the time of service unless other arrangements have been approved.

Approved discounts may need to be renewed periodically or sooner if household income, household size, insurance coverage, or financial circumstances change.

The Sliding Fee Discount Program does not replace insurance verification, medical-necessity requirements, prior-authorization requirements, or other payer rules.

Meadowlark Mind & Body is not a crisis center or emergency service. Billing forms, website forms, emails, texts, and portal messages are not monitored for emergencies.
